Role Description

Sentara Health Plans Roanoke is looking to hire a Care Coordinator- LPN. This position is remote work from home with travel to Nursing facilities required. The Care Coordinator performs a variety of casework duties and provides case management services to Members in Nursing Facilities, including patients, families, and designated caregivers.

• Develop, participate, and monitor multidisciplinary collaboration of services to patients where appropriate.
• Assist adult patients and their families with personal and environmental difficulties associated with medical conditions, including terminal illnesses.

Qualifications

• Associate's degree in Nursing (preferred).
• Bachelor's degree in Health & Human Services-related field or related experience in lieu of education accepted.
• Licensed Practical Nurse or Bachelor's prepared health professional strongly (preferred).
• LPN certification strongly (preferred).

Requirements

• MCO/HMO experience.
• 3 years of clinical experience within the last 5 years.
• 3 years of Medicaid/Medicare experience (preferred).
• Familiar with HEDIS measures (preferred).
• Healthcare experience (preferred).
• Case Management/Care Coordination experience (preferred).
• Experience with Community Health Resources (preferred).
• Experience initiating care plans and conducting assessments (both subjectively and objectively) (preferred).
