[Hiring] Team Lead-Centralized Clinic Support @Southcoast Health System

Role Description

Join Southcoast Health as a Team Lead-Centralized Clinic Support, where you will be at the heart of our community-focused and care-driven mission.

• Reports to the Manager or designee.
• Coordinate clinical support and personnel productivity monitoring.
• Provide ongoing training, guidance, and support to nursing and clinical staff.
• Ensure compliance with clinical workflows, policies, procedures, and best practice standards.
• Monitor daily performance reports to measure staff performance and identify improvement opportunities.
• Collaborate with team manager to build and maintain daily staffing schedules.
• Perform duties related to ambulatory nursing and clinical support programs, including:
• Telephone triage
• Care coordination
• Medication refills
• Addressing preventative health care gaps
• Other centralized services to support ambulatory patient care and population health management
• Implement and monitor clinical workflows, escalate clinical performance concerns, and follow-up on quality and patient safety concerns.
• Collaborate with Professional Development staff to onboard, educate, assess competency of, and monitor clinical staff.
• Ensure clinical practice of care to promote safe and high-quality patient care in collaboration with practice leadership.

Qualifications

• Minimum of five years nursing or relevant clinical experience required.
• Three years of ambulatory care nursing/medical office experience preferred.
• Bachelor's degree in Nursing or related field highly desirable.
• Supervisory experience preferred.
• Excellent interpersonal, communication, and leadership skills.
• Ability to manage multiple priorities.
• Computer skills including Microsoft Office and Electronic Health Record (EHR) experience required.
• Current RN licensure in the Commonwealth of MA and/or RI is required.
• Dual licensure required within 3 months of hire.
• Certification in Ambulatory Nursing, Care Management or Care Coordination and Transitional Care desirable.
• Current BLS certification is required.
• Must be fully vaccinated against seasonal Influenza and the COVID-19 virus or be exempt from the requirement.

Requirements

• Pay rate will be determined based on level of experience.
• Pay Range: USD $47.61 - USD $87.42 /Hr.
• Rotation Schedule Requirement: every other weekend / holiday.
