Role Description

The RN Care Transition Coordinator position is responsible for providing care transition services for Providence hospitalized patients. The Coordinator works with the Providence physician team and hospital discharge planning staff to ensure patients are successfully discharged from the hospital with no disruption in services. The position encompasses the roles of:

• Nursing assessment
• Liaison abilities
• Communication
• Education
• Team/family support

Qualifications

• Coursework/Training - Graduate from an accredited school of Nursing.
• Washington Registered Nurse License upon hire.
• Bachelor's Degree in Nursing (preferred).
• 1 year Clinical experience (preferred).
• Experience in providing patient and staff education (preferred).
• Experience in conflict resolution (preferred).

Requirements

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as:
