Role Description
The RN - Care Coordinator at Evernorth Workplace Care in Cleveland, OH plays a crucial role in delivering proactive, personalized, and holistic patient care. This position involves:

Identifying and referring appropriate customers to eligible programs within CIGNA Healthcare (CHC) and client-specific health and wellness programs.

Supporting care coordination for high-risk targeted populations in an efficient and cost-effective manner.

Acting as a central conduit between employers, providers (PCP, Onsite Health Clinic), and Evernorth programs.

Ensuring thorough review of patient-level actionable data for customer outreach and engagement.
-
Maintaining timely, compliant documentation to drive outreach and positive clinical outcomes.

Communicating clearly with matrix partners and referring to internal and external programs.
-
Identifying cases for complex case management and following referral processes.

Working effectively with Evernorth's internal team for care coordination and data analysis.

Building relationships with local network providers and referring to the onsite health center.

Identifying gaps in care and collaborating with providers to close them.
-
Establishing positive relationships with customers, employers, and Evernorth partners to improve outcomes.

Coordinating with health management partners for engagement and promotion opportunities.

Directing customers with non-clinical questions to appropriate resources.
Qualifications

RN with current unrestricted state licensure in Ohio.

Multistate licensure is preferred.

Three or more years of clinical practice experience.

Case Management experience is preferred.

High energy level with excellent oral and written communication skills.

Excellent computer skills (e.g., Word and Excel).

Strong interpersonal skills and ability to work in a team environment.

Commitment to ongoing education preferred.

Comfortable communicating with multiple clinical partners.

Experience working with high-risk patients preferred.

Experience in project development and working in a matrix environment preferred.

Self-directed with the ability to succeed in an independent role.

Current BLS/CPR/AED Certification.

Passion for assisting members with health improvement.

Strong problem-solving skills.
Requirements

CHES or NBHWC certification is a bonus.

Health Coaching experience including chronic disease management is a plus.
