[Hiring] Nurse Care Coordinator Manager @Clarity Pediatrics

Role Description

The Nurse Care Coordinator Manager (RN) is an integral member of Clarity Pediatrics' interdisciplinary care team, collaborating with physicians, advanced practice providers, psychologists, and therapists. Under the direction of clinical leadership, the Nurse Care Coordinator lead ensures seamless care delivery for children with chronic conditions by:

• Coordinating clinical appointments
• Providing patient and family education
• Managing medication authorizations
• Serving as a primary point of contact between families, providers, and pharmacies

This role will also be the supervisor for the team of nurses, medical assistants, and community health workers. Note this is a fully remote position.

Qualifications

• Active California Registered Nurse (RN) license in good standing (Bachelor's, or Master's degree in Nursing accepted)
• Minimum 10 years of nursing experience
• Experience leading a team of clinical staff
• Demonstrated proficiency with multiple technology platforms and electronic health record (EHR) systems
• Strong organizational, communication, and interpersonal skills
• Ability to manage competing priorities and work collaboratively in a remote, virtual environment
• Comfort working with diverse, medically and socially complex pediatric populations

• Reliable internet connection and a dedicated workspace suitable for remote work

Requirements

• Full-time position
• Regular work hours of M-F from 8 AM - 5 PM PT + on-call coverage for escalation of care by Care Coordinator Team
• $100,000 - $120,000/year with medical and ancillary benefits
