[Hiring] Nurse Practitioner - Telehealth @Loddon Mallee Health Network

Role Description

The Nurse Practitioner will function autonomously and collaboratively in an advanced and extended clinical role through the use of telehealth consultations with Residential In Reach teams and Residential Aged Care facility staff across the Loddon Mallee Region to provide high level clinical assessment support. The position can be based at locations throughout the Loddon Mallee region.

The Nurse Practitioner will deliver a video telehealth service that supports Residential In Reach teams and Residential Aged Care facility staff to support patient-centred care, operating within a nursing model of holistic practice and maintain a focus on best patient outcomes. The Nurse Practitioner provides non-urgent assessment and treatment of residents with acute medical or geriatric issues.

This role will provide casual cover to the position, and will include some weekend shifts.
