[Hiring] Contracted Case Manager @Morrison Center

Role Description

Morrison Center is seeking a Contracted Case Manager for our Brewer, Maine office. This is a REMOTE position and also to have hands on in person engagement. MUST BE IN MAINE! This is a temporary contract position for a defined term and does not constitute employment.

Qualifications

• Bachelor’s degree in any field (required)
• Minimum 1 year of experience working with adults in human services
• Experience with PCPs, family systems, and the Evergreen system is preferred
• Strong communication, time management, and problem-solving skills
• Ability to work independently with a high level of professionalism
• Proficient in Microsoft Office Suite

Requirements

• Candidates should possess relevant case management experience and meet all applicable professional requirements and qualifications.

Benefits

• Contract Type: Independent Contractor
• Pay: $65 per billable hour!
• Contract Duration: a 4-month independent contract engagement, July - October
• Schedule: Flexible hours (Monday–Friday)
• Location: Remote with travel for home/community visits in Maine

Responsibilities of the Community Case Manager

• Support adults with intellectual and developmental disabilities in achieving their personal goals
• Develop and manage Person-Centered Plans (PCPs), assessments, and service goals
• Ensure all documentation is accurate and compliant with Department of Health and Human Services requirements
• Conduct monthly home visits and maintain open communication with clients and families
• Perform research and collaborate with service providers to ensure effective supports are in place
• Serve as a trusted resource and advocate in the community
