[Hiring] Telehealth Therapist @Agape Behavioral Health

Role Description

The Telehealth Therapist is responsible for performing a wide variety of patient care activities as directed by the Clinical Director. He/she will be responsible for patient caseload and will provide group, individual, and family therapy. The Telehealth Therapist will conduct psychosocial and bio-psychosocial assessments and collaborate with the Treatment Team to formulate the Master Treatment Plan and assist in coordinating discharge planning.

• Review assessments from referring facilities and note problems/issues that require further attention.
• Contact referral source and other key individuals to gather additional information as necessary and maintain contact throughout patient’s treatment.
• Gather comprehensive diagnostic information through psychosocial assessment interview and develop an Integrated Summary.
• Develop a comprehensive problem list that reflects patient’s individual issues and present the psychosocial and diagnostic information to the treatment team.
• During the first session with the patient, review the initial treatment plan and ensure understanding of assignments and responsibilities.
• Formulate a comprehensive treatment plan with the patient that effectively addresses patient issues with measurable goals and objectives.
• Participate effectively in multidisciplinary treatment plan reviews, demonstrating adequate preparation and understanding of patient issues.
• Develop interventions that will accomplish goals and objectives and state the planned frequency of interventions.
• Maintain clinical records according to program policies and document the patient’s progress accurately.
• Continue treatment planning process throughout patient’s stay in treatment, revising treatment plans as necessary.
• Lead and co-facilitate group therapy sessions that create an atmosphere for self-disclosure and insight into problems.
• Complete activity and group plans for the Clinical Director including supplies utilized and/or needed.

Qualifications

• Masters Degree Required
• LCSW, LMFT, LMHC Required
• A minimum of 1 year experience in substance abuse or mental health treatment preferred.
