[Hiring] Remote Monitoring Hub Support Worker @Croydon Health Services NHS Trust

Role Description

The post holder will provide a high standard, efficient, cost-effective Remote Monitoring/Virtual Ward service to patients with a range of acute and chronic conditions hosted by Croydon Health Services NHS Trust in partnership with acute and community health service providers in Kingston, Richmond, Merton, Wandsworth, and Sutton.

• Support patient discharges from hospital.
• Prevent avoidable in-patient admissions.
• Reduce pressure on inpatient bed capacity while maintaining standards of care equivalent to those in hospital.
• Act as a support for the Remote Monitoring Hub/Virtual Ward team members in monitoring patients under the care of the Virtual Ward.
• Line managed by and work closely with the Remote Monitoring team to ensure an effective clinical service for the Virtual Ward Service.

Qualifications

• Moderate assessment skills.
• Ability to monitor and support patients via the Remote Monitoring/Virtual Ward.
• Strong leadership and communication skills.
• Motivated and innovative approach to support the development of the Remote Monitoring Hub.

Requirements

• Experience in providing care for patients with complex needs.
• Ability to work closely with local community teams.
• Early escalation of unwell patients.
