Role Description

The leading cardiology group in the West Valley is expanding and looking to add valuable and skilled team members to our comprehensive and long-standing Practice. Cardiac Solutions provides a personalized, team-oriented approach to patient care by promoting wellness through education, innovation, and technology. We offer intensive educational opportunities for patients through our disease management clinics and employ a multidisciplinary approach. All team members play a vital role in providing our patients with tailored care and support services. Our focus helps to prevent the progression of heart disease and minimize hospital admissions for our patients.

Qualifications

• Compassionate, professional, ethical, and knowledgeable approach to patient care.
• Ability to educate patients and their families on current conditions.
• Knowledge of dietary and lifestyle changes, medication therapy, and exercise.
• Experience in developing patient care plans.
• Ability to perform medication analysis and adjustments.
• Collaborative skills to work with leadership on patient care plans.
• Knowledge of cardiac nursing theory, techniques, principles, and practice.
• Familiarity with medical equipment.

Requirements

• Educates patients and families on their current condition.
• Develops patient care plans to monitor and maintain patient conditions.
• Performs medication analysis and adjustments based on testing.
• Continually assesses and adapts patient care plans.
• Collaborates with leadership for the best patient care plans.
• Follows policies for disposing of biohazardous materials.
• Maintains working knowledge of all medical equipment.
• Participates in interdisciplinary team meetings.
• Travels to other offices based on patient care needs.
• Other duties as assigned within scope of licensure.
