Role Description

Rapid Response/Urgent Crisis Response Clinician Band 6

• Work as part of a team of clinicians, therapists, rehabilitation assistants, and Community Geriatricians.
• Prioritize avoidance of unnecessary hospital admission.
• Assess and treat patients during a crisis, identifying complex and specialist needs.
• Develop, implement, and evaluate programmes of care.
• Facilitate early hospital discharge for patients back to their own home.
• Support and promote the ongoing development of all care delivery within the patient's own home.
• Work with a high level of autonomy, managing undifferentiated and undiagnosed problems using advanced clinical skills.
• Incorporate physical assessment and diagnosis.

Qualifications

• Relevant clinical qualifications.
• Experience in crisis response and patient assessment.
• Ability to work autonomously and as part of a multidisciplinary team.

Requirements

• Proficient in fitting appropriate technology for virtual ward monitoring.
• Responsible for assessing unwell patients at home on request of the virtual ward hub.
• Strong communication skills for interacting with patients and team members.
