[Hiring] Clinical Pharmacist Telepharmacy @CommonSpirit Health

Role Description

As our Tele-Pharmacist is based in our Houston market where you’ll help our doctors and nurses safely prescribe and dispense medications to help our patients feel better and recover from their illness or injury. This role will work, primarily, evening hours with the opportunity to cover shifts as they are available.

• Meticulously review every prescription in your queue for safety.
• Critically assess for potential drug interactions, correct dosing, patient allergies, or any irregularities.
• Authorize prescriptions for the pharmacy technician, performing a final check before dispensing medication to the nursing station.

• Review, verify, and interpret all physician orders received using the patient profile.
• Screen all medication orders for optimal medication usage and intervene when appropriate within the standards of the clinical programs and client hospital protocols.
• Monitor for medication allergies, drug interactions, renal and hepatic function, age-appropriate drug selection and dosage, duplicated orders, injectable compatibilities, concentrations, and administration rates.
• Use pharmacokinetic skills to determine dosing and to monitor patients’ responses to selected drugs under order of the prescriber or by approved medical staff protocols.
• Participate in continued education in pharmacy practice to meet regulatory requirements and organizational goals.
• Support and assist in meeting client hospital goals and specifications for order entry/verification, medication preparation, and clinical programs.
• Review, understand, and respond as appropriate in a timely manner to communication from System Director, Manager(s), and/or Supervisor(s) of Telepharmacy and from client hospital liaisons.
• Give thorough, concise communication reports at the time of order review or otherwise when appropriate detailing incomplete assignments and potential patient concerns to monitor to ensure continuity of medication therapy.

Qualifications

• B.S. in Pharmacy or equivalent from an accredited college of pharmacy.
• Three years of hospital pharmacy experience as a pharmacist.
• Active Texas Pharmacist license in good standing.
