[Hiring] Case Management Analyst @CVS Health

Role Description

As an Analyst, Case Manager, you support optimal member health outcomes by coordinating care, providing education, and facilitating access to appropriate healthcare services. Using care management tools and data analysis, you assess member needs, identify risks, and develop and implement effective care plans.

• Collaborate with internal teams, providers, and external resources to address barriers and improve quality of care.
• Ensure services align with member benefits.
• Promote member engagement, informed decision-making, and healthy behavior changes through communication, coaching, and motivational techniques.
• Monitor care plan progress, document activities, and ensure compliance with regulatory standards, company policies, and quality management guidelines.

Qualifications

• 2 or more years of behavioral health, social services, or human services field experience.
• 2 or more years’ case management experience.
• Must possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed by our company expense reimbursement policy.

Requirements

• Discharge Planning and/or Managed Care (MCO) experience preferred.
• Microsoft Office Suite experience preferred.
• Bachelor’s Degree or non-licensed Master’s level clinician required, in behavioral health or human services (psychology, social work, marriage and family counseling).
• Work from Home Requirements:
• Direct/hardwired internet connection to a modem/router within 7 feet of your computer.
• A minimum download speed of 25 mbps and upload speed of 3 mbps.
• Wi-Fi and satellite internet are not permitted.
• A quiet, secure, and private designated home virtual work location, compliant with CVS Health and HIPAA guidelines.
• Work-from-Home colleagues are required to work within the state and city where they have confirmed they currently live.
• The company will provide equipment (keyboard, monitor, computer, headset, etc.).
• All new hires should provide their own workspace furniture (desk or standing desk).
• Commit to obtaining required internet speeds and adhere to all Work from Home requirements.
• Technical and Logistical Requirements:
• Comfortable setting up and using multiple monitors and navigating multiple applications simultaneously.
• Ability to communicate on digital channels such as email, calendar invites, Teams messaging, and virtual meetings.
• Experience with Microsoft Office 365 (Teams, Outlook, Word, Excel, PowerPoint) applications or similar (Google Workspace).
• Ability to log in to secure systems (e.g., VPN, EHR portal) and manage strong passwords.
• Ability to resolve common technical issues independently.
• Openness to learning new skills in the future as the workplace environment evolves.
