Role Description
As an integral part of a team, the Care Coordinator is responsible for supporting logistical patient care activities for patients cared for under Mount Sinai at Home. This role is responsible for performing various remote duties involving, but not limited to:

Providing support and assistance with orders necessary for home-based care

Coordinating with vendors and deliveries
-
Ensuring patients receive necessary services based on care team orders
-
Administrative tasks requiring clear communication and collaboration with clinical team, patients, vendors
-
Sharing accountability for clinical quality and safety outcomes
Responsibilities include:
-
Working as a member of our interdisciplinary care team assessing and addressing the complex care needs of patients and families

Supporting admission and ongoing home-based census operations and following up on orders to coordinate care

Providing administrative support for tracking ongoing care activities (vendor liaison, equipment pickup coordination, appointment scheduling, discharge order confirmation, etc.)

Communicating plan of care and assessments to front line teams
-
Participating in huddles/IDT meetings daily/weekly with the rest of the care teams
-
Meeting with leadership on a regular cadence (minimum monthly) to discuss Mount Sinai at Home quality metrics, provide feedback and solutions on program workflow expansion

Managing and maintaining operational database including relevant care coordination and care delivery related metrics

Serving as a liaison between physicians, clinical staff, and vendors

Setting priorities, problem solving, and using proper judgment in difficult situations

Monitoring Epic orders

Promptly answering calls from clinical teams

Ensuring that patient’s privacy is protected by adhering to MSHS information security policies and procedures and HIPAA guidelines

Maintaining accurate records of interventions and safety events
-
Submitting utilization and assignment report every shift to supervisor and site leadership

Maintaining attendance/punctuality records in accordance to department procedure and/or policy

Other assigned duties/responsibilities
Qualifications
-
Minimum one year of healthcare experience including knowledge or experience with the following, three years preferred:
-
Experience with care coordination, patients, families, clinical staff
-
Ability to identify data elements to collect to support quality improvement/quality assurance
-
Experience drafting organizational protocols and workflows
-
Experience working with EMR, strong telephonic and coordination organization skills
-
Proven ability to identify strategies for measuring and communicating program successes and lessons learned
-
Education, Licensing and Certification Requirements (if applicable):
-
Associate’s or Bachelor’s degree in health-related field, required
-
Spanish speaking proficiency/fluency, preferred
-
Experience working as a healthcare paraprofessional, preferred

BCLS, Preferred
