Role Description

What it means to be a Behavioral Health Care Manager with Us:

• Support children and adolescents through assessment, care coordination, and ongoing behavioral health management.
• Partner closely with an interdisciplinary team and a psychiatric consultant to ensure young patients receive timely, effective, and integrated care.
• Perform duties in accordance with federal, state, and local regulations along with established priorities and procedures of the practice.

What a day looks like as a Behavioral Health Care Manager:

• Conduct pediatric behavioral health assessments (in-person or via telehealth).
• Develop and update individualized care plans for children and adolescents.
• Coordinate services with providers, schools, community agencies, and family supports.
• Communicate with families via phone, telehealth, and secure messaging.
• Collaborate with a psychiatric consultant and clinical team on pediatric cases.
• Maintain accurate documentation in the EHR.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Counseling, or related behavioral health field.
• Minimum 2 years of experience in pediatric behavioral health, mental health, or case management.
• Experience may include social work, counseling, or other behavioral health roles (licensure not required).
• Ability to effectively use phone, telehealth, and virtual communication tools.
• Working knowledge of computer applications; prior experience with electronic medical records preferred.
• Knowledge of pediatric mental health conditions, community resources, and trauma-informed care.
• Ability to speak, read and write effectively.
• Ability to work independently without direct supervision.
• Ability to coordinate, analyze, observe, make decisions, and meet deadlines, in a detail-oriented manner.

Requirements

• Support the mental and physical health care of patients on an assigned patient caseload.
• Screen and assess patients for common mental health and substance abuse disorders.
• Facilitate patient engagement and follow-up care.
• Provide patient education about common mental health and substance abuse disorders and the available treatment options.
• Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications.
• Support psychotropic medication management as prescribed by medical providers.
• Provide brief behavioral interventions using evidence-based techniques.
• Facilitate in-clinic or outside referrals to evidence-based psychosocial treatments as clinically indicated.
• Participate in regularly scheduled caseload consultation with the Psychiatric Consultant.
• Document patient progress and treatment recommendations in EHR and other required systems.
• Facilitate treatment plan changes for patients who are not improving as expected.
• Facilitate referrals for clinically indicated services outside of the organization.
• Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals.
