Role Description

This is a safety sensitive position. The Referral Coordinator is a member of the interdisciplinary medical team of the health center. The employee is responsible for working with the providers and the patients to ensure a smooth referral process.

Qualifications

• High school diploma, sometimes combined with medical assistant certification.
• Strong customer service focus.
• Effective verbal and written communication skills.
• Teamwork orientation.
• Good judgement and organizational skills to manage competing priorities.
• Resourcefulness in problem solving.
• Able to take and follow through with delegated tasks and accountability.

Requirements

• Contact review organizations and insurance companies to ensure prior authorization requirements are met.
• Present necessary medical documentation such as history, diagnosis, and prognosis information.
• Provide specific medical information to financial services to maximize reimbursement to the hospital and physicians.
• Review details and expectations about the referral with patients.
• Ensure referrals are addressed in a timely manner and follow up on patient referral inquiries not immediately resolved.
• Assist patients in problem solving potential issues related to the health care system, including financial or social barriers.
• Be the system navigator and point of contact for patients and families regarding their referrals.
• Identify and utilize cultural and community resources.
• Establish and maintain relationships with identified service providers.
• Remind patients of scheduled appointments via mail or phone.
• Ensure that patient's primary care chart is up to date with information on specialist consults, hospitalizations, ER visits, and community organization related to their health.
• Ability to work remotely at the discretion of the supervisor and in alignment with GSP Health policies.
