[Hiring] Program Coordinator @ENDEAVORS

Role Description

The Program Coordinator will work within the Endeavors Transition Support Program (TSP) with the primary responsibility of providing case management support to transitioning service members enrolled in Onward Ops.

• Establish and maintain supportive relationships with participants.
• Assess individual needs and identify appropriate community and VA resources.
• Facilitate connections to services that support successful transitions.
• Conduct Columbia-Suicide Severity Rating Scale (C-SSRS) screenings.
• Develop safety plans when indicated and provide ongoing follow-up and support.
• Document participant progress and coordinate referrals to behavioral health, wellness, and other community resources.
• Monitor participant engagement and identify barriers to accessing services.
• Collaborate with program partners to ensure participants receive timely and appropriate support.

Qualifications

• Bachelor’s degree preferred.
• Experience working with Veterans, Service Members, and their families.

Requirements

• Must maintain regular and acceptable attendance.
• Ability to remain on task in a work from home environment.
• Must be available and willing to travel to various locations as needed.
