Role Description

The Virtual Ward is an innovative service aimed at supporting people to receive care, support, and interventions within their own home. This improves the quality of care for individuals, reduces the length of time people stay in hospital, minimizes the need for hospital admissions, and ensures beds are available for acute admissions.

The successful applicant will provide clinical leadership and line management to the Virtual Services Team. They are expected to:

• Lead by example, motivate and empower team members to achieve their potential.
• Provide clinical leadership and line management to the staff.
• Maintain a visible, accessible, and authoritative presence in the Virtual Services Team.
• Create and maintain a learning environment that meets the educational and professional development needs of the staff.
• Work Trust-wide to promote the safe and effective utilization of virtual care pathways.
• Work across organizations and link with social services and Primary Care Teams.
• Support family and carers to ensure safe community-based care.

Qualifications

• A full Driving license is essential for this post.

Requirements

• Coordinate and manage the Virtual Services team.
• Ensure good working relationships and systems with other agencies.
• Use high-level communication skills to provide and receive complex information.
• Maintain a professional attitude at all times.
• Develop the role of the staff to the highest professional standards.
• Provide advice and information to staff ensuring consistent high standards of communication.
