Role Description

This position is remote and Per Diem. The primary purpose of your job is to be responsible for the delivery of patient care in accordance with current applicable Federal, State, and local standards, guidelines and regulations and established policies and procedures as may be directed by the Administrator, to assure that the highest degree of quality care and safety can be maintained.

Supervises: LPNs, CNAs, Medical Assistants.
Reports to: Nurse Manager or Practice Manager.

Essential Functions

• Embodies the mission, vision and values of Martha’s Vineyard Hospital and the Primary Care Department.
• Applies knowledge of illnesses, injuries and diseases and uses available resources in assessment process.
• Completes nurse visits as assigned.
• Documents assessment of patient at time of visit.
• Documents nursing intervention, patient response, effectiveness, complications, etc.; communicates information to provider.
• Implements clinical and technical aspects of care and nursing orders in accordance with established policies and procedures.
• Intervenes with proper technique, procedures, and safety precautions to meet individual needs of patient.
• Implements physician's orders.
• Manages patient care in department. Coordinates with providers, Practice managers, PSCs, Department Coordinators, and MAs and delegate tasks as needed.
• AED/O2 checks in the department.
• Manage incoming clinical needs faxes for department (routed from front desk), including but not limited to PAs, DME, VNA. Separate provider faxes and distribute to them.
• Works with assigned care team to coordinate care with outside agencies.
• Consults with all available in-house resource personnel and departments for treatment.
• Respects dignity and confidentiality of patient acting as an advocate.
• Provides a safe and secure environment for the patient.
• Performs in a calm and professional manner during crisis and/or emergency situations.
• Functions within limits of the State Nurse Practice Act.

Accountability: Patient and Family Instruction/Education

• Reviews patient and family knowledge of diseases, illness, or readiness to accept care for patient at home.
• Identifies needs for care, or alternatives such as outpatient treatment, extended care facility, etc.
• Sets realistic learning objectives by including patient and family in instruction planning.
• Provides self-care instructions.
• Identifies necessary medical information to be communicated to community resources, which are utilized.
• Uses individualized techniques designed to meet specific patient needs.
• Initiate referrals, as necessary, to healthcare providers, other resource agencies, etc.

Accountability: Quality of Work

• Adheres to nursing dress code and wears identification badge.
• Demonstrates organizational skills in providing nursing care by the ability to adapt to changing workloads.
• Performs the following according to policy:
  • Participation in plan of care with entire team.
  • Medication/immunization administration.
  • Nursing procedures.
  • Patient education.
• Maintains open communication channels with all colleagues and physician(s).
• Communicates factual and pertinent information in reports.

Accountability: Personal Growth and Development

• Identifies own learning needs and seeks opportunities for self-growth.
• Uses and shares skills gained at continuing education classes.
• Has Current BLS, initial or annual competencies completed.

Accountability: Age-Specific Competency

• Evaluates for age-appropriate behavior motor skills and physiological norms of patients.
• Recognizes patient's psychological development and modifies approaches for patients.
• Recognizes potential lifestyle changes resulting from illness that will interfere with the care plan and develops plan.
• Identifies increased physical/emotional dependency needs and evaluates availability of support system.
• Interview/communication techniques are age appropriate.
• Assesses patients for signs and symptoms of abuse/neglect and refers to appropriate personnel and/or community agency.

Risk Exposure Categories

• Tasks may involve exposure to blood/body fluids.
• Tasks do not involve contact with blood/body fluids but could result in performing a Category 1 task.
• Tasks do not involve any risk of exposure to blood/body fluids.

Working Conditions

• Sits, stands, bends, lifts, and moves intermittently during working hours.
• Is subject to frequent interruptions.
• Is involved with patients, residents, staff, government agencies/personnel, etc. under all conditions and circumstances.
• Is subject to hostile and emotionally upset patients, family members, etc.
• Communicates with the medical staff and other hospital departments’ staff.
• Attends and participates in continuing training programs as they relate to the position.
• May be exposed to infectious waste and diseases.
• Is subject to lifting, carrying, and supporting patients.
• Must be constantly alert for patient safety.

Qualifications

• Bachelor's Degree Nursing required or Associate's Degree Nursing required.
• Can this role accept experience in lieu of a degree? No.

Requirements

• Registered Nurse [RN - State License] - Generic - HR Only required.
• Basic Life Support [BLS] - Generic - HR Only required.
• A minimum of one year experience as an RN 0-1 year preferred and Physician office experience preferred and New grads will be considered.

• Must be able to read, write legibly, speak clearly, and understand the English language.
• Must possess the ability to make independent decisions when circumstances warrant such action in a timely fashion.
• Must possess the ability to deal tactfully with staff, patients, family members, visitors, government agencies/personnel and the public.
• Must possess the willingness to work harmoniously with professional and non-professional personnel and the public.
