Role Description

SynergenX is seeking a full-time Telemedicine Provider to care for patients throughout Indiana via telehealth. This is a fully remote, full-time W-2 position.

• Provide comprehensive treatment for both men and women, including hormone management and medical weight loss.

Qualifications

• Active Indiana Family Nurse Practitioner (FNP) license or Physician Assistant license.
• Current Indiana CSR and FULL DEA registration (required at the time of application).
• Current National Provider Identifier (NPI) number.
• Current board certification applicable to your profession.
• No current or pending malpractice lawsuits.
• Experience or strong interest in hormone replacement therapy, women's health, preventive medicine, or medical weight management is preferred.
• Bilingual English/Spanish is highly preferred.

• Exceptional patient communication and relationship-building abilities.
• NPI number and board certification information must be disclosed during the application process.
