Role Description

As an RN Health Coach, you will support your patients on their wellness journey by:

Providing customer coaching and support, via video and telephonically, with a focus on stress, anxiety, depression and other challenges.

Performing stress, anxiety and depression screening.

Identifying customer health education needs through targeted health assessment activities.

Referring customers to other Cigna Evernorth clinical programs or client specific programs as clinically appropriate.

Collaborating with customers to establish health improvement plans, set personalized evidence-based goals, and support customers in achieving those goals.

Empowering customers to become active participants in their own health outcomes.

Assisting customers in overcoming barriers to better health.

Leading and supporting a variety of health and wellness promotional activities, such as group coaching, wellness challenges and health-related seminars.

Utilizing biometric values and motivational interviewing techniques to collaborate with customers to drive to improve clinical outcomes.

Collaborating with our matrix partners to support the overall health and wellness strategy for your aligned client(s).

Understanding and owning a variety of clinical targets and outcome measurements.

Developing action plans that drive clinical value for the customers and clients.

Providing support in training or mentorship of new employees as needed.
Qualifications
-
Strong clinical skills with at least 1 or more years of experience in health coaching, health education and health promotion.

RN required with active, unrestricted license in respective state, and ability to maintain.

Compact license or additional state licenses may be required, with ability to maintain.

Current ACLS/BLS/CPR/AED Certification.

Minimum of an Associate’s Degree in Nursing; Bachelor’s Degree strongly preferred.

At least 3 or more years of RN experience in a hospital or provider’s office setting.

High-speed internet and quiet office space in committed location.
-
High energy level, with dynamic presentation skills is required.

Requirements
-
Behavioral Health experience, including stress reduction (bonus points).
-
Master’s degree in psychology or related field is preferred (bonus points).

Chronic Disease Management experience (bonus points).
-
NBHWC (National Board-Certified Health & Wellness Coach) (bonus points).

Motivational interview training/experience (bonus points).
-
Additional languages including Spanish (bonus points).
