[Hiring] RN Case Manager @Pyramid Consulting

Role Description

Immediate need for a talented RN Case Manager. This is a 06+ months contract opportunity with long-term potential and is located in U.S (Remote).

Key Responsibilities:

• Conduct outreach to newly assigned and existing members to complete required assessments of member needs.
• Develop individualized care plans based on clinical, behavioral, and social needs, continuously reassessing as member needs evolve.
• Connect members to primary care providers, specialists, and community resources, facilitating scheduling and referrals.
• Provide education and support to ensure compliance with treatment plans, monitoring adherence to improve health outcomes.
• Identify and close HEDIS care gaps to enhance preventive care and chronic condition management.
• Maintain performance metrics by managing caseloads, completing assessments, and ensuring compliance with care plans.

Qualifications

• Experience in care management with a focus on chronic health conditions.
• Background in managed care and case management.
• Bachelor's degree in Nursing.
• Certification / License in RN Licensure (Required).
• 2-4 years of experience in case management and/or managed care.
• Proficiency in Microsoft Office Suite and strong documentation skills.
• Excellent communication and engagement skills to build rapport and educate members.
