[Hiring] Patient Access & Intake Coordinator @Kindred Healthcare

Role Description

The Patient Access & Intake Coordinator plays a key role in facilitating a smooth and efficient patient admission process. This position provides vital administrative and operational support to the Business Development team and the Central Access and Authorizations Team (CAAT), ensuring timely initiation and management of referral activity.

• Responsible for launching the referral management process.
• Oversees admission workflows, gathers and verifies required documentation, and coordinates information for necessary approvals.
• Communicates essential information to relevant departments.
• Acts as a central liaison between Business Development, facility leadership, and CAAT colleagues.
• Coordinates admission logistics and confirms readiness to receive the patient.
• Ensures all clinical and operational details are aligned prior to arrival.
• Contributes to quality improvement, problem solving, and productivity initiatives within an interdisciplinary team model.

Qualifications

• Team player, able to communicate and demonstrate a professional image/attitude.
• Excellent oral and written communication and interpersonal skills.
• Strong computer skills with both standard and proprietary applications.
• Data entry with attention to detail.
• Must read, write, and speak fluent English.
• Must have good and regular attendance.

Education

• High School Diploma or GED required.
• Associates or Bachelors Degree preferred.
• Preference towards a healthcare related area of concentration or be a licensed health care provider or equivalent experience.

Experience

• Experience in a healthcare related area is preferred.
• Work experience in post-acute care a plus.
