Role Description

As an LPN Healthcare Advocate with Solace, you'll work with Medicare patients throughout their healthcare journey. In the LPN Advocate role, you will:

• Guide patients through difficult and complex health concerns.
• Help them achieve their health and wellness goals while addressing their Social Determinants of Health (SDOH).
• Be an empathetic listener and an action-oriented guide who resolves patients' problems.

PLEASE NOTE: this is a 1099 contract position. You will set your own hours and can work part-time, full-time, or anything in between. This role is 100% remote. All patient interactions are performed via video or phone call.

What You’ll Do:

• Learn Solace's proprietary system, tools, technology, and expectations, while providing your unique experience in every interaction.
• Build strong, trusting relationships with patients, where listening and empathy are the foundation for every interaction.
• Identify and prioritize patients’ needs and build out a care plan to ensure consistent action over time.
• Develop comprehensive patient care plans that holistically address social determinants of health, i.e. food resources, transportation access, and support at home.

Qualifications

• LPN license in good standing.
• Experience working with diverse patient populations.
• Empathy: You care deeply about people and have a drive to fight for those who can't fight for themselves.
• Clinical and Organizational Skills: You can balance multiple tasks and work under pressure.
• Technical Adaptation: You're able to learn new systems quickly and efficiently.
• A Bias Toward Action: When you see something needs to get done, you step in and take care of it.
