[Hiring] Epic Healthy Planet Architect @ConsultNet

Role Description

A large healthcare organization is separating its payer and provider operations into distinct entities, with the new provider organization going live in Epic on January 1. We are seeking a senior Epic Healthy Planet professional who can advise at the strategy level and stay hands-on in the build. You will design the end-to-end workflows that move payer data into Epic, surface open care gaps to providers at the right point in their workflow, and return documented actions to the payer to close gaps and update quality metrics.

• Assess current Healthy Planet configuration and provider workflows, and design future-state processes for population health, care management, and care gap closure.
• Architect bidirectional payer-to-provider workflows so incoming payer data lands in the right place in Epic and is actionable in ambulatory and population health workflows.
• Partner with interface and integration teams on FHIR activation, API connectivity, and third-party bridge solutions between payer and provider systems.
• Advise leadership on Healthy Planet and Compass Rose best practices, registry design, and how to structure population health operations across separate organizations.
• Build and optimize registries, care gap logic, quality measures, and outreach workflows supporting HEDIS and value-based care programs.
• Ensure provider actions are captured and transmitted back to the payer to close gaps and trigger downstream revenue cycle processes.
• Support go-live readiness, training, and post-live stabilization for the new provider organization.

Qualifications

• 5+ years of Epic experience with a focus on Healthy Planet.
• Active Epic Healthy Planet certification.
• Hands-on registry build, care gap management, and quality measure configuration experience.
• Proven workflow design and optimization experience across ambulatory and population health.
• Strong understanding of provider operations and how clinicians act on data within Epic.
• Ability to operate as both a strategic advisor and a hands-on builder.
• Excellent stakeholder communication with clinical, operational, and technical leaders.

Requirements

• Compass Rose certification or hands-on experience.
• FHIR, API, and Epic interoperability experience, including payer-provider data exchange.
• Care Management and Care Coordination workflow build.
• HEDIS, quality reporting, and risk stratification experience.
• Cogito / Clarity reporting knowledge.
• Health plan or payvider experience, ideally both payer and provider perspectives.
• Prior consulting experience on Epic implementations or major go-lives.
