Role Description
Icon Health is seeking a board-certified Family Medicine physician with fellowship training in Sports Medicine to serve as a collaborating physician for our Advanced Practice Provider team. This role is intended to support licensing, credentialing, and collaboration requirements under Tennessee law. The physician must live and practice physically in the state of Tennessee and be willing to participate in required collaboration activities in a timely, responsive, and partnership-oriented manner.
The collaborating physician will work with Icon Health’s Physician Assistants and Nurse Practitioners under Icon Health’s Practice Agreement, which defines the medical services APPs are authorized to perform based on their demonstrated competency, education, training, and experience. APP services may include:

Patient care activities

Signing appropriate forms

Supervising medical assistants and care coordinators

Prescribing medications appropriate to the APP’s level of training, expertise, and applicable law
The collaborating physician is not required to be physically present while APPs provide medical services; however, they must be available by telephone or other electronic means when APPs are providing services, consistent with Tennessee requirements and Icon Health’s collaboration guidelines.
Qualifications

Board certified in Family Medicine

Fellowship trained in Sports Medicine

Actively licensed and in good standing in Tennessee

Physically located and practicing in the state of Tennessee

Comfortable serving in a collaborative physician capacity for APPs

Responsive, organized, and willing to move quickly to support credentialing submissions and CAQH profile updates

Aligned with Icon Health’s mission to improve orthopedic access, outcomes, and patient experience through a collaborative, value-based care model
Requirements

Serve as a collaborating physician for Icon Health APPs practicing in Tennessee

Support licensing, credentialing, CAQH updates, payer enrollment, and related administrative requirements

Be available by telephone or other electronic means when APPs are providing medical services, as required by applicable law and the Practice Agreement

Provide clinical collaboration, guidance, and consultation when APPs encounter patient care needs that exceed their competency, education, training, or experience

Support appropriate referral or escalation to a physician or other healthcare professional when clinically indicated

Participate in Icon Health’s APP competency and qualification evaluation process, including peer review activities based on the applicable standard of care

Collaborate with Icon Health to ensure APPs have the qualifications, training, and experience necessary to perform authorized medical services, procedures, and medication/device ordering or furnishing activities

Review and approve the Practice Agreement on a regular basis and support updates when warranted by changes in conditions, circumstances, regulations, or clinical operations

Sign required collaboration, credentialing, and regulatory documentation in a timely manner
