Role Description

The Health System Command Center Clinical Pharmacy Manager provides centralized clinical pharmacy services across the health system through a virtual command center model. This pharmacist supports inpatient, hospital-at-home, and ambulatory care environments by delivering:

• Remote medication order verification
• Medication reconciliation
• Clinical consultation
• Tele-pharmacy services

The role serves as a system-wide clinical resource, improving medication safety, supporting hospital capacity initiatives, and enhancing care transitions by ensuring accurate medication management across multiple sites of care.

This position plays a key role in supporting Hospital-at-Home programs, providing:

• Real-time remote medication verification
• Patient counseling
• Clinical decision support
• Care coordination for care teams

Remote pharmacists commonly review medication orders, monitor therapy, and intervene with providers to prevent medication-related problems across multiple facilities from a centralized hub.

Qualifications

• Pharmacy degree from an accredited institution
• Active pharmacist license
• Experience in clinical pharmacy practice
• Strong communication and interpersonal skills
• Ability to work in a fast-paced environment
