Pay Range: $38/hr - $42/hour.

Key Responsibilities:

• Review referrals and risk stratification reports to identify care needs.
• Conduct comprehensive trimester-based assessments and develop individualized care plans.
• Coordinate services with providers and community partners, ensuring seamless care transitions.
• Engage in member outreach to address gaps in care and document interventions in the care management platform.
• Collaborate with interdisciplinary teams to support high-risk pregnancy management and postpartum engagement initiatives.
• Monitor compliance with regulatory standards and track performance metrics related to quality and member engagement.

Qualifications

• 3+ years of recent RN clinical experience.
• 5+ years of nursing experience in care management, maternal-child health, OB, NICU, pediatrics, population health, Medicaid, managed care, or community health.
• Active, unrestricted RN license required.
• Strong critical thinking and clinical assessment skills.
• Exceptional communication and motivational interviewing abilities.
• Highly organized and self-directed with a passion for improving maternal and infant health outcomes.
• Experience in case management.
