[Hiring] Clinical Aged Care Needs Assessor @Loddon Mallee Health Network

Role Description

The Clinical Aged Care Needs Assessor is responsible for conducting comprehensive assessments for older people within the Aged Care Planning Regional area of Victoria (VIC 3 – Loddon Mallee). This role forms part of the My Aged Care Single Assessment Workforce, delivering high-quality, person-centred assessments that support older Australians in accessing appropriate aged care services.

• Face-to-face needs assessments requiring travel within the Loddon Mallee and/or remote assessments via telehealth.
• Provides clinical expertise and guidance to Non-Clinical Aged Care Needs Assessors.
• Ensures assessments are accurate, holistic, and aligned with relevant aged care standards and policies.
• Opportunity to undertake other functions such as delegation and triage.

Qualifications

• Dependent on qualification - as per Allied Health or Nursing EBA.
