Role Description

The Clinical Aged Care Needs Assessor is responsible for conducting comprehensive assessments for older people within the Aged Care Planning Regional area of Victoria (VIC 3 – Loddon Mallee). This role forms part of the My Aged Care Single Assessment Workforce, delivering high-quality, person-centred assessments that support older Australians in accessing appropriate aged care services.

• Face-to-face assessments requiring travel across the region and/or remote assessments via telehealth or phone.
• Work in the Intake/Triage area.
• Provide clinical expertise and guidance to Non-Clinical Aged Care Needs Assessors.
• Ensure assessments are accurate, holistic, and aligned with relevant aged care standards and policies.
