Role Description

Trillium Health Resources has a career opening for a Care Transitions Specialist II (NE) – My Choice to join our team! The Care Transitions Specialist II (NE) supports individuals transitioning from institutional and residential settings to community-based care. The role coordinates services, provides education and transition planning, and addresses barriers to ensure safe, timely, and successful transitions to integrated community settings. This Care Transition Specialist II will serve as a member of the My Choice program for IDD members currently living in an intermediate Care facility and or State Developmental Center.

On a typical day, you might:

• Coordinate care transition activities to support timely movement between care settings.
• Execute transition plans based on established guidelines and assessed needs.
• Communicate with providers and partners to ensure services are in place during transitions.
• Monitor transition timelines and ensure required actions are completed.
• Facilitate referrals and confirm service connections following transitions.
• Provide education on services and resources within the community.
• Link individuals to community resources and services for community-based integration.

Qualifications

• Bachelor's degree in human services and two (2) years of experience working directly with complex individuals with IDD/MH/SU or TBI.
• The following immunizations are required for this position:
  • Fully vaccinated against Covid-19, Influenza (annually within Influenza season Nov 1 - March 31), MMR, Varicella and Tdap (Tetanus, diphtheria pertussis).
  • MMR and Varicella - must show proof of vaccination or have blood drawn to show immunity.
  • Tdap - must show proof of one dose.
  • Employees must provide proof of all immunizations prior to their start date or agree to provide proof of all immunizations within 4 months of start date.
• Must have a valid driver’s license.
• Must reside within Trillium’s Northern, North Central, or South Central Region, which includes the following counties: Bertie, Camden, Chowan, Currituck, Gates, Halifax, Hertford, Martin, Northampton, Pasquotank, Perquimans, Warren, Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrrell, Washington, Wilson, Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, and Wayne.
• Must be able to travel within catchment as required.
• Must be legally authorized to work in the United States without requiring sponsorship for employment now or in the future (ex: OPT F-1, J-1).
