Role Description
We are currently seeking a Nurse Practitioner to join our virtual clinic supporting Central Time Zone from 9 am to 5 pm Monday through Friday.
As a Nurse Practitioner at Heartbeat Health, youβll play an integral role in delivering high-quality, patient-centered cardiovascular care through our virtual care model. This position is ideal for an experienced provider who is passionate about cardiology and comfortable independently managing complex cardiac patients in a remote environment.

Conduct thorough patient assessments via telehealth visits, including medical history reviews, physical examinations, and diagnostic tests, to evaluate cardiovascular health and identify cardiac conditions.

Formulate differential diagnoses and develop comprehensive treatment plans for patients with various cardiac issues. Prescribe medications and order diagnostic tests.

Extensively manage chronic cardiovascular conditions remotely, with a heavy emphasis on advanced Heart Failure management, hypertension, arrhythmias, and coronary artery disease. Monitor patient progress, analyze real-time data, and adjust care pathways to optimize cardiac function and prevent hospital readmissions.

Expertly implement, titrate, and manage Guideline-Directed Medical Therapy (GDMT) for heart failure and alternative complex cardiac populations, ensuring clinical practice aligns strictly with the latest ACC/AHA guidelines.

Educate patients and their families about cardiovascular diseases, prevention strategies, and lifestyle modifications. Provide specialized counseling on medication adherence, GDMT titration expectations, fluid/diet management, exercise, and smoking cessation.

Collaborate closely with cardiologists, PCPs, and other healthcare professionals to ensure comprehensive and coordinated care for patients. Discuss complex cases and participate in multidisciplinary care teams.

Stay current with the latest developments in cardiology research and incorporate evidence-based practices into patient care. Participate in research studies or quality improvement initiatives to enhance patient outcomes.

Maintain accurate and detailed patient records, including medical histories, assessments, treatment plans, progress notes, and discharge summaries. Ensure compliance with legal and ethical standards.

Serve as an advocate for patients' needs and preferences, ensuring that they receive appropriate care and support throughout their healthcare journey.

Pursue ongoing education and professional development in the field of cardiology to stay updated on the latest advancements and maintain relevant certifications.

Participate in quality assurance programs to monitor and improve the quality of cardiology care provided within the healthcare facility.
Qualifications

A Master of Science in Nursing (MSN) with a specialization in cardiology or a related advanced practice nursing field.

3+ years of dedicated experience working as a Nurse Practitioner in an outpatient cardiology setting or clinic environment.

Must hold an active, current, and unrestricted Nurse Practitioner (NP) license (preferably in a Compact State).

Obtain and maintain Nurse Practitioner (NP) certification specific to cardiology from a recognized certifying body, such as the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners Certification Board (AANPCB), in a relevant Nurse Practitioner specialty (e.g., Adult-Gerontology Acute Care Nurse Practitioner or Family Nurse Practitioner with a strong focus on cardiology).

Demonstrated proficiency in outpatient management, interpreting complex diagnostic tests (e.g., ECGs, echocardiograms), and optimizing clinical outcomes for advanced Heart Failure populations.

Proven expertise in the practical application, optimization, and protocol titration of Guideline-Directed Medical Therapy (GDMT).

Certification in ACLS is required or highly recommended, as Cardiology Nurse Practitioners may be involved in emergency cardiac care protocols.

Excellent communication skills to effectively educate patients virtually and collaborate with a multidisciplinary healthcare team, including cardiologists and remote care coordinators.

Ability to make critical decisions, analyze complex cardiac cases, and rapidly adapt treatment plans based on patient responses and changing clinical conditions.

Compassion and empathy when working with patients and their families, especially when delivering challenging diagnoses or managing chronic, life-altering cardiac conditions.

Meticulous attention to detail in patient assessments, virtual documentation, and treatment plans to ensure medical accuracy and patient safety.

Familiarity with electronic health records (EHR) systems, remote patient monitoring (RPM) tools, and other digital healthcare technologies used in virtual cardiology practice.
