Role Description
The role involves care delivery, planning of care, therapeutic relationships, communication, and documentation and data management.

Care Delivery:
-
Develop and implement individual crisis and safety plans with youth and family.
-
Facilitate referrals and linkages, monitor care plan implementation.
-
Manage 1915(c) Waiver funds ensuring prior authorization and ODM approval.
-
Ensure effective referrals and linkages with appropriate assessments, support, and services.
-
Respect youth and family’s rights to define goals and choose providers.
-
Facilitate member outreaches, assessments, and meetings in person or virtually.
-
Operate at a ratio of ICC-1:10 and MCC-1:20.
-
Integrate comprehensive and evidence-based strategies into CFT meetings.
-
Demonstrate knowledge of behavioral health management skills.

Plan of Care:
-
Implement/document patient response to individualized care plans.
-
Participate in regular team planning meetings as needed.
-
Engage newly referred youth and family in care coordination.
-
Develop Child and Family-Centered Care Plan (CFCCP) with family.
-
Facilitate Child and Family Team (CFT) meetings at least monthly.
-
Complete comprehensive assessments including CANS assessments.
-
Engage youth and family in forming their unique CFT.
-
Ensure CFT meetings are convenient for the family.

Implement trauma-informed care principles and culturally responsive approaches.

Therapeutic Relationships/Communication:
-
Coach and develop new staff, enhancing quality in care planning.
-
Engage with system of care peers for adherence to the model.
-
Facilitate CFT, ensuring collaborative CFCCP development.
-
Coordinate care with all providers and agencies involved.
-
Communicate ongoing CFCCP implementation progress to Team members.
-
Develop community resources to meet youth and families’ needs.
-
Utilize culturally competent and trauma-informed care principles.
-
Documentation and Data Management:
-
Complete patient care plan documentation accurately and timely.
-
Bill for care coordination activities.
-
Ensure quality information management and documentation.
-
Ensure written CFCCP is signed and placed in youth’s record within 2 weeks.
-
Provide CFCCP to Aetna for quality review and authorization process.
-
Develop SMART goals with clear measures for achieving goals.
Qualifications
-
Bachelor’s degree and 2+ years’ experience or Master’s degree with 1 year experience in one or more of the following areas:

Family systems
-
Community systems and resources
-
Case management
-
Child and Family counseling or therapy
-
Child protection
-
Child development
-
May require clinical license, certification, or training.
