Role Description

The Assessment Specialist conducts Home Enabling Supports (HES) assessments and develops person-centered HES Plans for individuals with intellectual and developmental disabilities (I/DD). Working under the direction of the HES Program Development Manager and closely with individuals, families, Care Managers, and vendors, the Assessment Specialist identifies opportunities to promote independence, safety, health, and community participation through assistive and enabling technology. The position is responsible for:

  • Completing assessments
  • Identifying appropriate technology solutions
  • Developing HES Plans
  • Ensuring required documentation meets OPWDD and program standards

Assessments may be conducted in the individual's home or remotely.

Qualifications

  • At least one (1) year of experience conducting technology assessments, including direct or supervised experience assessing, implementing, or supporting technology-based solutions for individuals
  • One of the following:
    • A credential in enabling or assistive technology, such as Enabling Technology Integration Specialist (ETIS) or Assistive Technology Professional (ATP)
    • -OR-
    • A clinical background with at least two (2) years of experience working with individuals with intellectual and/or developmental disabilities in a related field, such as Occupational Therapy, Physical Therapy, Speech Therapy, Rehabilitation Counseling, or Social Work
  • Experience with person-centered planning and service delivery preferred
  • Strong communication, interpersonal, organizational, and documentation skills
  • Ability to work independently and collaboratively with individuals, families, Care Managers, vendors, and other stakeholders
  • Ability and willingness to travel throughout ADAPT's service area, including the five boroughs of New York City, for in-home assessments and related activities
  • Commitment to ADAPT Community Network's mission and to supporting individuals with intellectual and developmental disabilities to live as independently and fully as possible

  • Maintain accurate and timely records and complete required documentation within established timeframes
  • Participate in activities that support effective program operations, quality, and compliance
  • Perform other related duties as assigned
