Role Description

The Azalea Health Analyst serves as a key member of the Population Health and Value-Based Care team, supporting Accountable Care Organization (ACO) initiatives through the administration, optimization, analysis, and reporting of the Azalea Health Population Health platform. This role partners with clinical, operational, quality, care management, and executive stakeholders to leverage data, improve patient outcomes, identify care gaps, support quality performance programs, and maximize success across value-based reimbursement arrangements.

Qualifications

• Experience in population health or value-based care.
• Strong analytical skills and proficiency in data analysis.
• Ability to collaborate with diverse stakeholders.
• Knowledge of healthcare operations and quality performance programs.

• Contract position based out of Philadelphia, PA.
• Fully remote position.
