Role Description

Trillium Health Resources has a career opening for a Peer Support Specialist to join our Support Transition and Reentry (STAR) team. The Peer Support Specialist provides support to recipients enrolled in the Trillium STAR program, which serves adults experiencing behavioral health (BH) or intellectual/developmental disabilities (IDD) conditions who are exiting incarceration.

• Educates members and guardians about navigating the criminal justice system and accessing community-based services.
• Works with the recipient’s care team to provide transition linkages including:
  • School
  • Employment
  • Housing coordination
  • Community service opportunities
  • Provider services
  • Other identified treatment supports and services
• Establishes relationships with area justice systems, including local county jails, prisons, and probation and parole officers.
• Ensures recipients are well connected and provides a warm hand-off to identify needed treatment and support.
• Meets with justice-involved members in-person in correctional facilities and in the community to facilitate care management activities.
• Engages members who meet criteria for Transitions to Community Living (TCL) to provide education about permanent supportive housing and community resources.
• Assists with transition planning and ensures delivery in the most effective manner with a focus on optimal quality outcomes.
• Utilizes person-centered planning methods to gather information and address specific needs of individuals.

Qualifications

• High School Diploma or GED.
• One (1) year lived experience with a behavioral health condition and/or substance use.
• North Carolina Peer Support Specialist Certification.
• Must have a valid driver’s license.
• Must reside in North Carolina and within 75 miles of Trillium’s North Central Region, specifically Pitt County.
• Must be able to travel within catchment as required.

Requirements

• Preference given to applicants with a history of criminal justice system involvement, incarceration, or mental health/substance use recovery.
• Education, training, and work experience that demonstrates the ability to work with diverse individuals.
