[Hiring] Partnership Lead @Change Grow Live

3d ago - Change Grow Live is hiring a remote Partnership Lead. 💸 Salary: £45,583.51 - £47,850.69 📍Location: UK

Role Description

We are looking for a proactive and influential Partnership Lead to help shape the future of health and social care integration. In this strategic role, you will build and strengthen partnerships across Integrated Care Systems, Combatting Drugs Partnerships, criminal justice agencies, and health and social care providers. Working closely with local services, you will provide expert guidance, identify opportunities for collaboration, and support the delivery of innovative, joined-up approaches that reduce drug and alcohol-related harm, improve treatment outcomes, and tackle health inequalities.

Responsibilities

  • Map, monitor and understand the local Integrated Care System (ICS) and related activity.
  • Map, monitor and understand the local Combating Drug Partnership (CDP) strategy and outcomes framework.
  • Work with the service data lead to help coordinate data analysis to feed into ICS and CDP developments.
  • Take on project management of any partnership developments including partnership projects funded through additional funding grants, and work with the services manager to appropriately allocate and develop funding areas.
  • Build and sustain healthy relationships with local partners and respond in a solution-focused way to challenges.
  • Develop relationships with CDP and sub-groups, Primary Care Networks including Clinical Directors, local place-based partnerships, Mental Health Transformation Teams, Acute trusts and VCSE alliance.
  • Upskill staff and teams by providing information, advice, and guidance, that unpacks the complexities and enables services to integrate across criminal justice and the health and social care system.
  • Work as part of the service leadership team to monitor, capture and report on all OHID grant funded activity and provide any overview update to Service(s) Manager and commissioner reporting.
  • Apply a strength-based and system-focused approach to working, sharing best practice and cascading any learning from positive service activity.
  • Educate local leaders about the value of the service on a ‘human’ level and to recognise the purpose and benefits of integrated working.

Qualifications

  • Experience of working within substance misuse services.
  • Experience of managing projects.
  • Experience working with vulnerable adults in a health and social care setting.
  • Knowledge and understanding of the possible pathways available within the Health, Social Care and Voluntary sector.
  • Able to develop, design new methods and approaches to update practices and service delivery.
  • Key project management skills and ability to set targets and meet key deadlines.
  • Able to lead and embed a solution-focused approach to dealing with challenge and conflict in integrated working.
  • Able to use a data-driven approach to influence partnership relationships, improve service provision and to tell our story.
  • Able to communicate (at many levels) performance using a variety of data and demonstrate ability to summarise key outputs.

Benefits

  • Enjoy a dedicated weekly Wellness Hour, access to our Wellness Hub, and a comprehensive Employee Assistance Programme to support your mental and emotional health.
  • We’re committed to your development, with extensive training, funded learning, and pathways into senior or specialist roles.
  • Benefit from transparent pay, a contributory pension scheme, and additional support through our wellbeing resources.
  • Start with 25 days of annual leave plus bank holidays (pro rata), increasing to the maximum 30 days over five years of service.
  • Join a supportive, values-led team where you’ll feel encouraged and valued from your very first day.
  • Where possible, we’re open to discussing working patterns that help you maintain a healthy work–life balance.
  • Enjoy discounts on shopping, cinema trips, holidays and more, along with our Refer a Friend scheme.
