Role Description

Wellness Coordinators function in a non-clinical capacity responsible for providing care coordination or administrative support to members with behavioral health and substance abuse needs.

• Act as a primary care navigator by locating and engaging members.
• Screen for and identify gaps in care.
• Address SDOH needs and develop individual care plans to overcome barriers to access to care.
• Provide administrative support to the Behavioral Health Case Management department through assigning referrals, providing chart summaries, and verification of member demographics.

Schedule: Full-time, 40 hours per week, Monday through Friday, from 8:00 AM to 5:00 PM CST. Work schedules may vary based on business needs and coverage requirements. Flexibility to work hours between 8:00 AM and 8:00 PM in your local time zone and/or members' time zones may be required.

Location: Remote National. You’ll enjoy the flexibility to telecommute from anywhere within the U.S. as you take on some tough challenges.

Qualifications

• High School Diploma
• 1+ years of call center experience
• 1+ years of managed care experience
• 1+ years of computer proficiency experience

Requirements

• Bachelor degree in behavioral health, psychology, social work or related field (preferred)
• 1+ years of experience in Medical/Behavioral health setting (i.e. hospital, managed care organization, or joint medical/behavioral outpatient practice) (preferred)
• Casework experience at hospitals, social service agencies or community mental health centers (preferred)
• Dual diagnosis experience with mental health and substance abuse (preferred)
• Experience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients (preferred)
• Working knowledge of community-based resources and services (preferred)
• Creative problem solving skills with the ability to use community and network resources to meet members' needs (preferred)
• Demonstrates initiative in achieving individual, team, and organizational goals and objectives (preferred)
