Role Description
Wellness Coordinators function in a non-clinical capacity responsible for providing care coordination or administrative support to members with behavioral health and substance abuse needs.

Act as a primary care navigator by locating and engaging members.

Screen for and identify gaps in care.

Address SDOH needs and develop individual care plans to overcome barriers to access to care.

Provide administrative support to the Behavioral Health Case Management department through assigning referrals, providing chart summaries, and verification of member demographics.
Schedule: Monday through Friday, a 40-hour work week between the hours of 8:00 am to 5:00 pm standard business hours in the time zone related to the work (market) (EST or CST).
Location: Remote Nationwide.
Qualifications

High School diploma.

1+ years of computer proficiency experience.

1+ years of Dual diagnosis experience with mental health and substance abuse.

1+ years of experience handling sensitive issues with members and providers in a confidential manner.

2+ years of experience in behavioral health and case management experience.

Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI).

Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.

Ability to work a 40-hour work week in the time zone related to the work (market) (EST or CST).

Experience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients.

Working knowledge of community-based resources and services.

Creative problem-solving skills with the ability to use community and network resources to meet members' needs.
