Role Description

As our Virtually Integrated Care RN, you will be a pivotal provider, empowering patients toward optimal health through comprehensive remote support and collaborative care coordination. You will be instrumental in bridging care gaps and enhancing engagement in a dynamic virtual environment.

• Meticulously assess patient needs and develop personalized care plans.
• Provide direct clinical coaching and proactively monitor progress.
• Coordinate diverse services and leverage telehealth.
• Diligently track compliance, document interactions, and contribute to quality improvement.
• Collaborate virtually with physicians, nurses, and other healthcare team members.
• Proactively oversee the patient care plan to implement preventive measures.
• Drive risk reduction through participation in root cause analysis (RCA’s) and quality surveillance.
• Compile and interpret large data sets of patient information.
• Collaborate with unit leadership and provide oversight of unit quality programs and metrics.
• Ensure evidence-based practices are followed by team members.
• Support management to ensure compliance with internal policies and regulatory requirements.
• Assess services, patient outcomes, quality control, and family/patient satisfaction continuously.
• Identify the need for changes in standards, programs, and services.
• Monitor processes in collaboration with bedside staff and provide recommendations for care delivery improvement.
• Participate in the development of strategies, initiatives, and programs addressing standardization of procedures and enhancing patient outcomes.

Qualifications

• Bachelors of Nursing (BSN) or actively pursuing a BSN degree upon hire.
• Minimum of 3 years of overall acute care nursing experience.
• Minimum of 2 years of nursing preceptor experience.
• 1-3 years in a nursing leadership role (Charge RN role applicable).
• Registered Nurse: AZ.
• Basic Life Support (BLS).
