Role Description

As our Virtually Integrated Care RN, you will be a pivotal provider, empowering patients toward optimal health through comprehensive remote support and collaborative care coordination. You will be instrumental in bridging care gaps and enhancing engagement in a dynamic virtual environment.

• Meticulously assess patient needs, develop personalized care plans, and provide direct clinical coaching.
• Proactively monitor progress, coordinate diverse services, and leverage telehealth.
• Diligently track compliance, document interactions, and contribute to quality improvement.
• Collaborate virtually with physicians, nurses, and other health care team members in the provision of clinical care.
• Assess virtually the medical, physical, and psychological status of patients by reviewing records and monitoring diagnostic studies.
• Establish a primary plan of care through an interdisciplinary team approach and ongoing patient assessments.
• Respond to routine, urgent, and emergent patient problems appropriately.
• Facilitate a collaborative spirit assuring teamwork by providing direction to staff.
• Monitor and provide feedback on performance of employees as requested by Nurse Director.
• Prevent patient problems through direct observation via telemedicine technology and proactive surveillance.
• Communicate effectively with members of the bedside interdisciplinary team to provide safe care.
• Coach and mentor nurses and nursing support on the floor around patient care.
• Advocate on behalf of patients and their families regarding their cultural, spiritual, and care needs.
• Virtually educate patients and families on disease processes, procedures, and self-care.

Qualifications

• Bachelors Of Nursing - Actively in pursuit of a BSN degree upon hire.
• Minimum of 3 years of overall acute care nursing experience, upon hire.
• Minimum of 3 years of nursing preceptor experience, upon hire.
• Minimum of 2 years of nursing leadership role (charge RN role applicable), upon hire.
• Registered Nurse: AZ, upon hire.
• Basic Life Support - CPR, upon hire.

Requirements

• Within 12 months: Masters Of Nursing - Clinical Nurse Specialist (CNS) accepted, upon hire.
