[Hiring] Virtual Ward Senior Coordinator / Practitioner @Midlands Partnership University NHS Foundation Trust

Role Description

We are looking for an enthusiastic, innovative and engaging Senior Practitioner who has a passion for the delivery of high-quality patient care within the community working as part of our Virtual Ward based in Tamworth, Lichfield and Burntwood. This is an internal fixed term position for 8 months, to cover maternity leave.

Our General Acute Medicine and Frailty Virtual Ward is a new and innovative Service, that has an agreed admission criteria and works in partnership with supporting care services to deliver acute care in the patient’s own home. The aim of a virtual ward is to:

• Reduce length of acute hospital stay and/or avoid hospital admission.
• Lead to quicker recovery and reduce disruption to patients’ and their carers’ lives.
• Proactively manage people living with frailty at home.
• Improve a patient’s experience and outcomes for those with sub-acute episodes.
• Free up space in hospitals for acutely unwell patients and reduce pressure on urgent care.

As part of the Virtual Ward Team, you will have delegated responsibility for the clinical management of patients that have been stepped up or stepped down onto the Virtual Ward, which includes:

• Assessment of care needs.
• Development, implementation, and evaluation of programmes of care.
• Setting of standards on a shift basis.

This is a part-time role, covering shifts over a 7-day period.

As the Senior Practitioner, you will take a leading role in the clinical assessment of people in hospital or in their own home environment, who have been identified as meeting the Virtual Ward admission criteria. As part of this role, you will be expected to:

• Participate in innovation and evidence-based nursing practice through professional leadership.
• Support the Virtual Ward clinical team in close liaison with the Advanced Clinical Practitioner and the Virtual Ward Clinical Lead (Quality).
• Be responsible for the Virtual Ward Nursing Associates and the Virtual Ward HealthCare Support staff on shift.
• Ensure that the patient remains the center of all decision-making.
• Prioritize own workload without supervision.
• Maintain an excellent patient-focused approach and use initiative while working as part of a multidisciplinary team.
• Discuss digital assessments with the patient and implement an agreed digital solution where possible.
• Ensure that staff on shift are regularly monitoring and reviewing in-line with the patient care plan.
• Hold a full UK driving licence and have access to a vehicle for work-related travel.
