Role Description

SynergenX is seeking a full-time Telemedicine Provider to care for patients throughout Colorado via telehealth. This is a fully remote, full-time W-2 position.

• Maintain a balanced patient population with an approximately 50/50 focus on men's hormone replacement therapy (MHRT) and women's hormone replacement therapy (WHRT).
• Evaluate, treat, and manage patients through individualized hormone optimization, medical weight management, and preventive healthcare designed to improve overall health, wellness, and quality of life.

Qualifications

• Active Colorado Family Nurse Practitioner (FNP) license with Colorado APRN licensure (Family specialty only) or an active Colorado Physician Assistant license under your primary taxonomy NPI.
• Current Colorado DEA registration (required at the time of application; no company reimbursement is provided).
• Current National Provider Identifier (NPI).
• Current board certification applicable to your profession.
• No current or pending malpractice claims or disciplinary actions.
• Experience or a strong interest in hormone replacement therapy, preventive medicine, men's health, women's health, and medical weight management is preferred.
• Bilingual English/Spanish is a plus.

• Excellent patient communication and relationship-building abilities.
• NPI number and board certification information must be disclosed during the application process to be considered.
