[Hiring] Senior Pharmacy Care Coordinator @UnitedHealth Group

Role Description

Under the supervision of a licensed pharmacist, this role supports high-quality, compassionate patient care while adapting to the dynamic healthcare environment. Responsibilities include:

• Obtaining and reviewing post-discharge medical records
• Navigating multiple electronic medical record systems (EMRs)
• Analyzing clinical documents such as labs, history & physicals, provider notes, and discharge summaries
• Conducting outreaches to members to update medication lists
• Coordinating with providers to confirm care relationships
• Following up on clinical recommendations
• Identifying medication discrepancies
• Reviewing prescription claims data
• Preparing detailed reports for pharmacist reviews
• Collaborating across care teams to support coordination efforts
• Working independently in a fast-paced, evolving setting

Qualifications

• High School Diploma/GED
• Current, clear, and unrestricted Pharmacy Technician licensure in Utah
• Possess a National Certification (PTCB)
• 2+ years of experience in pharmacy roles
• Intermediate level of proficiency in the Microsoft Office suite, including Outlook, Teams, and Excel

Requirements

• Retail pharmacy experience (preferred)
• Experience with EMRs (preferred)
