Role Description

Supports complex case management activities to coordinate patient services, resource access, and continuity of support across multiple touchpoints.

• Coordinates appointments, referrals, authorizations, and related service needs to reduce access barriers and support timely case progression.
• Resolves escalated patient and caregiver inquiries related to coverage, benefits, documentation, and support pathways to improve case outcomes.
• Maintains accurate case records, status updates, and supporting documentation to strengthen traceability, compliance, and operational visibility.
• Connects patients to community resources, financial support options, and service programs to address practical barriers affecting access and adherence.
• Collaborates with healthcare providers, payers, pharmacies, and internal support teams to align case actions and close service gaps.
• Contributes to workflow improvement, knowledge updates, and standard work adoption to support scalable and future-ready case operations.

Qualifications

• High School Diploma/GED or equivalent experience required.
• Associate’s degree in healthcare administration, health sciences, social services, or a related field preferred.
• 3+ years of experience in case management, patient support services, customer operations, healthcare coordination, social services, or a related field required.
• Certification in case management, patient services, healthcare support, or customer service operations preferred.
