Role Description

As a Virtual Triage Nurse, you will be responsible for conducting comprehensive clinical assessments via telehealth platforms, prioritising patient care based on clinical urgency, risk, and presenting needs. You will play a key role in coordinating appropriate care pathways, ensuring patients are directed to the right service at the right time.

You will also provide patient education, support self-management strategies, and contribute to positive health outcomes through effective communication and clinical guidance.

• Conduct virtual clinical assessments, prioritising patients based on urgency, risk factors, and presenting symptoms.
• Obtain and accurately document patient histories, clinical information, and care plans within electronic health records.
• Coordinate referrals, diagnostics, and care pathways in collaboration with GPs, Nurse Practitioners, specialists, and allied health providers.
• Provide patient education and follow-up to support chronic disease management, self-management, and continuity of care.
• Work collaboratively within a multidisciplinary virtual care team while contributing to quality improvement and safe, compliant clinical practice.

Qualifications

• Registered Nurse with a Bachelor of Nursing (or equivalent).
• Current unrestricted AHPRA registration.
• Minimum of 5 years’ full-time experience as a Registered Nurse in Primary Health.
• Experience managing complex and chronic health conditions within a primary healthcare setting.
• Experience in immigration detention or similar environments highly regarded.
