Role Description

Here’s how you will support your patients on their wellness journey:

• Provide customer coaching and support, via video and telephonically, with a focus on Diabetes education and GLP-1 education.
• Perform stress, anxiety, and depression screening.
• Identify customer health education needs through targeted health assessment activities.
• Refer customers to other Cigna Evernorth clinical programs or client-specific programs as clinically appropriate.
• Collaborate with customers to establish health improvement plans, set personalized evidence-based goals, and support customers in achieving those goals.
• Empower customers to become active participants in their own health outcomes.
• Assist customers in overcoming barriers to better health.
• Lead and support a variety of health and wellness promotional activities, such as group coaching, wellness challenges, and health-related seminars.
• Utilize biometric values and motivational interviewing techniques to collaborate with customers to drive to improve clinical outcomes.
• Collaborate with our matrix partners to support the overall health and wellness strategy for your aligned client(s).
• Understand and own a variety of clinical targets and outcome measurements.
• Develop action plans that drive clinical value for the customers and clients.
• Additional responsibilities in alignment with the scope of your certification or licensure.
• Provide support in training or mentorship of new employees as needed.

Qualifications

• Strong clinical skills with at least 1 or more years of experience in health coaching, health education, and health promotion.
• RN required with active, unrestricted license in respective state, and ability to maintain.
• Compact/multi-state license is required, with ability to maintain.
• Additional state licenses may be required, with ability to maintain.
• Certified Diabetes Educator.
• Current ACLS/BLS/CPR/AED Certification.
• Minimum of an Associate’s Degree in Nursing; Bachelor’s Degree strongly preferred.
• At least 3 or more years of RN experience in a hospital or provider’s office setting.
• High-speed internet and quiet office space in committed location.
• High energy level, with dynamic presentation skills is required.
• Positive role model in demonstrating healthy behaviors.
• Passion for health improvement.
• Ability to work independently.
• Customer-centric focus.
• Ability to proactively collaborate professionally with the client and other matrix partners.
• Proven administrative abilities, with strong computer and software application skills.
• Adaptable and flexible to changes of a growing team.
• Ability to cover multiple time zones if needed.

Requirements

• Behavioral Health experience, including stress reduction (bonus points).
• Master’s degree in psychology or related field is preferred (bonus points).
• Chronic Disease Management experience (bonus points).
• NBHWC (National Board-Certified Health & Wellness Coach) (bonus points).
• Motivational interview training/experience (bonus points).
• Additional languages including Spanish (bonus points).

• Annual salary of 77,500 - 129,100 USD / yearly, depending on relevant factors, including experience and geographic location.
