Role Description
This role involves providing patient care support for a centralized nursing program, outpatient, and home care services. This includes Triage services, Anti-coagulation, and refill. Individuals in this role will work in an outpatient clinic setting.

Collects, prioritizes, and synthesizes comprehensive data pertinent to the patient's health or situation.

Analyzes assessment data to determine nursing diagnoses.

Identifies expected outcomes individualized to the patient.

Develops a plan that prescribes interventions to attain expected outcomes.

Implements the identified plan in a safe, timely, and appropriate manner.

Evaluates the patientβs progress towards attainment of the outcome.

Participates in quality improvement activities related to nursing practice.

Attains knowledge and competency that reflects current nursing practice.

Evaluates oneβs own nursing practice in relation to professional practice standards and regulatory guidelines.

Contributes to the professional development of peers, colleagues, and others.

Collaborates with patient, family, and others in the conduct of nursing practice.

Acts in an ethical manner and maintains a therapeutic and professional patient-nurse relationship.

Integrates research findings in practice.

Provides leadership in the professional practice setting and the profession.

Practices in an environmentally safe and healthy manner.

Demonstrates ability to coordinate and direct unit operation when acting as Charge Nurse.

Demonstrates ability to identify the orientee's learning needs when acting as Preceptor.

Qualifications

Associate's or Vocational degree in nursing

Minimum 3 years RN experience

Proficiency in set up and usage of computer technology including Microsoft Teams, Word, Excel, and SharePoint

Licensed Registered Nurse-MN Board of Nursing required

Licensed Registered Nurse-WI Dept of Safety & Professional Services required by completion of orientation
