As a PRN RN Case Manager at Avardis, you will take the lead in shaping best practice initiatives that enhance health outcomes, streamline access to care, and lower costs for the population we serve. Your role includes:

• Providing clinical leadership for Managed Care, Bundle Payment, and ACO programs.
• Acting as an educator and advocate for both patients and staff.
• Collaborating across departments to evaluate and direct activities that support patient care delivery.
• Overseeing data collection for quality indicators and identifying successes and areas for improvement.
• Developing comprehensive care plans with input from patients, families, and healthcare teams.
• Ensuring smooth transitions from inpatient care to the next level.
• Conducting proactive post-discharge follow-ups and consultations with physicians.
• Eliminating barriers for patients and enhancing healthcare delivery systems and results.

Qualifications

• Must possess a Degree in Nursing as a Registered Nurse.
• Must have a minimum of two (2) years' experience in a hospital, nursing care facility, or other related health care facility.
• Must have a working knowledge of managed care rules, regulations, and guidelines.
• Must have comprehensive knowledge of managed care, health insurance, and Case Management.
• Ability to negotiate coverage and provide complete and timely case management, bundled payment, and ACO reports.
• Prior experience with a Health Plan, private or third-party case management company preferred.
• Must be qualified, compassionate, and dedicated to a job well done.

Requirements

• Valid Degree in Nursing as a Registered Nurse.
• At least two years of experience in a hospital, nursing care facility, or similar healthcare setting.
• Comprehensive knowledge of managed care rules, regulations, and guidelines.
• Strong negotiation skills to ensure coverage and timely completion of case management reports.
• Experience with a Health Plan or third-party case management company is preferred.
