Role Description

We are looking for an RN to deliver patient-centered health coaching and chronic disease management via chat, phone, and video. This role includes:

• Triage symptoms and provide education on diabetes, hypertension, hyperlipidemia, and weight management.
• Coordinate care with providers and cross-functional teams.
• Document patient care accurately in a telehealth setting.

Our Registered Nurse Clinical Care Specialists provide direct care and education to patients with chronic cardiometabolic conditions, working closely with our Providers.

Qualifications

• Strong empathy, communication, and patient-centered care skills.
• Bachelor of Science in Nursing (BSN) required.
• Active Registered Nurse license in good standing, with ability to obtain additional licensure if needed.
• Certified Diabetes Care and Education Specialist (CDCES) or willing to pursue if not currently certified.
• 3+ years of direct patient care in an ambulatory or telehealth setting.
• Adaptable, self-directed, and comfortable working independently.
• Proficiency with digital tools for care delivery.
• Flexibility to participate in rotational weekend or holiday coverage.
• A HIPAA compliant, distraction-free workspace.

Requirements

• This role offers a salary of up to $100,000, depending on experience.
